Ursuline Transcript Requests

The Ursuline Corporation will maintain the Registrar’s Office (transcripts) on the Santa Rosa property.

Requests can be emailed to transcripts@ursulinehs.org

or sent through the US Postal Service to:
Registrar’s Office for UHS
90 Ursuline Road
Santa Rosa, CA 95403

Please include the following:
Your name as listed on your Diploma
Graduation Year
Your current mailing address, phone number and email address
Name and Address of where you wish your transcript to be sent.
